Application
How Long Does The Application Process Take?
The application process typically takes 2-4 weeks from initial submission to approval. We strive to process applications as quickly as possible while ensuring all requirements are thoroughly met. It may take longer if additional documentation is needed.

What Are The Requirements To Become A Provider?
Complete the 4 step process explained on Apply. Upon start, you are required to complete specific trainings to meet state-mandated compliance requirements. Refer to the section “Training” for additional information.

We require the following trainings and credentials:
- Level 1 fingerprint authorization card.
- CPR and First Aid certifications (Offered In-house!)
- Direct Care Worker (DCW) Training & Test
- Prevention and Support Training
- Habilitation (HAH) Training
- Cultural Competence Article 9 certifications (Offered In-house!)

Training
What Trainings are Offered by Growth abILITY sERVICES?
Growth Ability Services provides CPR/First Aid, Article 9, and Cultural Competency trainings in-house. These trainings are offered to support Direct Service Providers (DSPs) in meeting state-mandated compliance requirements. The agency will retain original certificates for all in-house trainings as part of the employee’s record. Growth Ability Services assist DSPs with the registration process for the external trainings that are not currently offered.
What Is The Cost Of In-House Training?
In-house trainings are usually free of charge.
The agency will retain original certificates for all in-house trainings as part of the employee’s record. You may request the release of training certificates for personal use; however, such releases will incur a fee. This fee is solely for administrative processing related to releasing the certificate and does not represent a charge for the training itself. Certificates will only be released upon receipt of the corresponding fee.
CPR/First Aid Certificate: $60
Article 9 Certificate: $45
How can i obtain the other required trainings?
Growth Ability Services does not provide certain required trainings and credentials directly but can assist DSPs with the registration process for these external trainings. In these cases, DSPs must pay the full cost of the training up front. Upon payment, the agency will register the DSP for the selected class. It is the sole responsibility of the DSP to attend the training and pass all required components. If the DSP fails to complete or pass the training, they will be responsible for covering the full cost again should they choose to be re-registered through the agency.
Fingerprint Clearance Card: $75
Direct Care Worker (DCW) Training: $100
Direct Care Worker (DCW) Test: $25
Prevention and Support: $60
Habilitation (HAH) Training: $20
*All prices listed are subject to change. The current cost will be confirmed and disclosed to the DSP prior to payment.
How Long Does Training Take To Complete?
Training times vary depending on the certificate. Most trainings can be completed in just a few hours or within a couple of days.
CPR & First Aid: 4–6 hours
Article 9: 3 hours
Cultural Competency: 1–2 hours
Direct Care Worker (DCW): 2 levels completed over 2 days (1 day per level)
Prevention & Support: 2 hours
HAH Training: 2–4 hours
Most participants complete their required trainings within a short timeframe.

What Are Qualifying Disabilities?
Qualifying conditions include:
- Intellectual (Cognitive) Disability
- Cerebral Palsy
- Epilepsy
- Autism Spectrum Disorder
These conditions must be diagnosed by state qualified professionals in Arizona and supported by appropriate documentation.
What Are The Typical Work Hours And Schedules?
You can create a mutual agreement with the member you’re working with. This allows for a flexible schedule that adjusts to your needs without a 9 to 5 schedule. This is one of our many benefits!
Do You Provide Benefits For Providers?
Absolutely! The possibility of having control over their own schedule, competitive rates, and weekly payday!
General
Do I Have To Pay For DDD Services?
Services are mostly free or covered by Medicaid for eligible individuals. However, if you’re not ALTCS-eligible, you might receive limited services or be asked to contribute based on your income. Please note this is all based on the state of Arizona’s findings.
