Pay for Training
Use this form to pay for required trainings. Growth Ability Services offers CPR/First Aid and Article 9 trainings in-house at no charge. For external trainings and credentials not provided in-house, payment must be submitted in full before the agency will complete your registration.
Request Certificate Release and Pay
Use this form to request the release of certificates or credentials on file, including CPR/First Aid, Article 9, Fingerprints, DCW, and HAH certificates. Please note that release of in-house training certificates may incur an administrative processing fee.
EVV Adjustment Request
Per AHCCCS and DDD, Direct Service Providers (DSPs) are required to utilize the EVV System to ensure, track, and monitor timely service delivery and access to care for members.
